Student Forms

Overview

Included on the pages following are forms for undergraduates, and coterminal, and graduate students. Petitions and procedures on the pages following are administered by the Office of the University Registrar. Policy regarding these forms and procedures can be found in the Stanford Bulletin and relevant pages throughout the Registrar's web site.

Submitting Forms

Forms can be submitted either in-person or electronically.

In-person submission

Completed forms, including all required signatures, may be dropped off at the Student Services Center, Tresidder Union, second floor.

Electronic submission

Completed forms, including all required signatures,  may be sent via the email address that is published on the form. The Student Forms Email Addresses page has a list of the email address for each form.

Late submission

Forms submitted after the published deadline require additional documentation to be considered for review.

Beginning the fourth week of the quarter, any submitted special registration status form or degree progress form listed below requires supporting documentation from the student to be considered for review. Requests must demonstrate exceptional circumstances for the late submission to be approved.

Leave of Absence forms are not accepted late if the student has already been discontinued for no enrollment. In order to be readmitted for the term, undergraduates must consult Undergraduate Advising (UAR), and graduate students must consult their home department.
