Procedures to Extend/Terminate a Student of New Faculty Appointment

The Student of New Faculty should inform the Stanford department administrator of their intent. It is the responsibility of the Stanford department administrator to complete the Students of New Faculty Appointment Extension/Termination (pdf) form. This form must also be signed by the Office of the Vice Provost for Graduate Education (VPGE). The complted form should be sent to Graduate Admissions, Office of the University Registrar. Visa related questions about cancellations should be directed to Bechtel.

The Students of New Faculty Appointment Extension/Termination (pdf) form is used to communicate any of the following:

  • Ending the appointment early (e.g., student graduated).

  • Cancelling the appointment (e.g., the student did not come).*

  • Extending the appointment for additional quarter(s) or for the next academic year.*

* If the request is to cancel or end the appointment early and the student has Cardinal Care Health Insurance:Confirm if the student has been charged Cardinal Care health insurance. If yes, it is the department administrator's responsibility to contact Vaden and have the health insurance fee removed. This must be done prior to submitting completed Students of New Faculty Appointment Extension/Termination (pdf) form to Graduate Admissions, Office of the University Registrar.
