Important Information Regarding COVID-19

For quickest response to an issue, please submit a ServiceNow Ticket to the Student Services Center. If you prefer to talk to an SSC Specialist, we offer advising sessions via Zoom by appointment. SSC specialists are also available for live chat from LiveChat Monday – Friday, 12:00 – 4:00 p.m. via the Service Now portal.

Student eForms

eForms is the platform that provides student access to and submission of Registrar-related and enrollment-based petitions. Both new electronic forms and existing pdf forms are housed there. By Spring 2021, this platform is scheduled to replace all pdf petitions with electronic forms that are routed to the Office of the Registrar for final approval.

See below for a table of all student forms submitted to the Registrar's Office or the Student Services Center. Also see below for instructions on submitting pdf forms.

To go to Stanford eForms, log in to Axess, hover over the Student mega menu, and select “Student eForms”.

On the eForms landing page, click on "Browse Available Forms" in order to submit a petition or form.

eForms only displays the forms that are available for your open programs (i.e., undergraduate, coterm, or graduate programs).

Students can track the progress of their forms using the information inside bubbles displayed in many locations in the eForms portal. The bubbles indicate the office(s) needed for approval of the form. Bubbles in green mean that the office has reviewed and approved the petition, and has now moved it to the next step in the workflow. The final step is approval or denial by the Office of the Registrar.

Students, faculty, or staff with questions about forms or procedures are advised to consult the Student Services Center, Tresidder Union, second floor, or to submit a SU Services & Support Request.

Submitting PDF Forms

Pdf forms must be submitted electronically.

Electronic submission

Completed forms, including all required signatures, may be sent via the email address that is published on the form. The Student Forms Email Addresses page has a list of the email address for each form.

Late submission

Forms submitted after the published deadline require additional documentation to be considered for review.

Any special registration status form submitted past the listed deadline will require supporting documentation from the student to be considered for review. Requests must demonstrate exceptional circumstances for the late submission to be approved.

Leave of Absence forms are not accepted late if the student has already been discontinued for no enrollment. In order to be readmitted for the term, undergraduates must consult Undergraduate Academic Advising, and graduate students must consult their home department.
